First, in general, you will get more replies if you post in the main HOD board. These boards are rarely checked outside of the top one.
As for grades, I don't keep grades. Our state doesn't require it. If my kids don't do at least B quality work, I make them redo it. So, the lowest they could earn is a B. And honestly it isn't worth it at this point. I will start keeping grades before too long though - starting with math. We use Horizons, so I will take the grades from the test. I will probably do grammar as well, using the review from Rod and Staff (or buy the tests for them).
In the high school guides, Carrie has a template ready in the introduction (you can see them as free PDFs). I know the WG one starts on Page 12 of the PDF.
